My dear Poppy i have been in a similar situation but the gentleman gave me another chance after explaining why i chose the name i saved him under, it was not the same name as yours but it was not the coolest name to save him under. That's why i don't save any client no anymore.

Ask him(your client) to call you so that you 2 can discuss face to face, and explain to him honestly why you saved him with that name.

And don't forget the magic word "SORRY". I gather that he is important client to you otherwise you will not post this.
