May I highlight an insight-

To offer OWO and CIM (as he is asking for) is an extra risk, which means Doctor's Fees should also be accounted for... on top of the SP's probable venue rate she has to pay, probable travel rate and the money she hopes to make from providing this service. It can be deduced that this would leave someone performing this risky behavior so for little money in a very vulnerable position.

I for one do not judge, but have immense compassion for any SP in this situation xxx
