Username changed?

So i couldn't log in this morning on my normal login details I've been using for years. Did a password reset thing and when i received my email it showed my username changed. How did that happen? Is this an Esa admin way of banning someone from their account or a weird ass bug? Username was always Ulquiorra it's now Ulquiorra###
